Position Summary

 

This position provides case management and stabilization services to formerly homeless clients with a disability living in subsidized housing. Assesses need for all main stream services including financial, legal, medical, mental health and substance abuse services and links to appropriate resources.  Generates referrals for vacancies within the program by providing outreach to homeless provider agencies, particularly to other hopeFound programs.

 

 Essential Functions:

  • Provide housing stabilization services including home visits
  • Develop and monitor individual service plans
  • Maintain program participant records, including progress notes, individual service plans and statistical information
  • Assess need for supportive services and link to internal and external resources, including but not limited to medical care, public benefits, and

mental health and addiction treatment services.

  • Maintain occupancy rate of over 90%; provide outreach as necessary
  • Assess clients’ housing needs, eligibility and housing-readiness
  • Assist clients with housing search when required
  • Maintain and develop relationships with private market landlords and realtors
  • Collaborate with program administrator for program evaluation
  • Participate in a 24 hour on-call emergency coverage rotation
  • Generate monthly statistics for housing programs
  • Assist in completing program reports
  • Attend staff meetings and supervisions
  • Seeks and participates in professional development opportunities 

 

Qualifications:

Required

§         Bachelor’s degree in human services or LADC/CADAC certification

§         Minimum of three years of experience in providing case management and/or advocacy services to a culturally diverse and disadvantaged population

§         Minimum of three years experience providing supervised substance abuse services

§         Knowledge of issues facing homeless, traumatized clients

§         Knowledge of treatment options and resources in Greater Boston

§         Strong verbal and written communication skills

§         Computer literacy required; familiarity with database preferred

§         Must have access to a vehicle, a valid Massachusetts driver’s license, and the ability to travel to and access scattered sites related to the housing programs.

 Preferred

§         Master’s in social work, human services, related field, or LMHC

§         Knowledge of harm reduction philosophy and methodologies

§         Knowledge of motivational interviewing

§         Training and/or experience providing services to racial, cultural and ethnically diverse populations

§         Master’s in Human Services

§         Bilingual (English / Spanish)

 

Physical/Mental Abilities Required:

  • Ability to handle and prioritize multiple tasks
  • Ability to provide crisis management
  • Ability to provide intensive case management
  • Ability to use computer
  • Ability to generate monthly statistics for housing programs and assist with APRs and other reporting requirements
  • Ability to travel to and access scattered sites related to the housing programs
  • Ability to act as liaison with landlords and realtors
  • Ability to problem solve

 

 

Salary Range:  $38,000-40,000 plus benefits, commensurate with qualifications
